Players that do not meet these requirements will not be permitted to participate. Should you not be able to provide a current copy of two of these three bills, please contact us.
Athletic Participation Forms
Fully completed forms, including a current physical valid through the 2011 season is required. Should your player have these forms on file for a different sport, it is your responsibility to obtain and submit copies of these forms, including a valid physical.
